 Our office does not accept any insurance. American Express, MasterCard, and Visa credit cards are accepted. Care Credit is also available. Care Credit is a medical charge card that works almost like a credit card and is only accepted where Care Credit is offered. Patient may fill out an application in our office or apply online www.carecredit.com Depending on the patients credit background, credit limit varies. Since we do not process any insurance claims, our office will provide a copy of the visit’s charge sheet for patients to submit to their insurance company (all medical codes needed are listed on charge sheet).

 The initial visit is $300.00/hour (US) not including blood work or other laboratory tests. Blood work and laboratory tests Dr. Starr deems necessary will be of additional cost. Typically, laboratory tests to evaluate hormonal status are an additional $400.00 (US). More extensive testing may be required depending upon the complexity of the case and the number of problems that need to be addressed. The first visit usually lasts about one and a half hours. Please have patient information forms completed prior to your initial appointment. Follow-up visits are $150.00 (US) not including prescriptions, supplements, or follow-up lab tests. Patients must have their initial evaluation performed in our office. Thereafter, phone consultations and annual examinations may be an option for out of town patients.

Injections and Physical Therapy:
 Injection costs vary. Muscular, ligamentous, or injections for painful scars are $250 (US) dollars including the pre-injection anesthetic block. Nerve blocks for painful conditions such as shingles are $200 (US) dollars. Physical therapy following injections is $150 (US) dollars per session.
